Dry Socket After Tooth Extraction: Signs, Prevention & Treatment

What Dry Socket Looks Like and Feels Like

Dry socket (alveolar osteitis) is the most common complication after tooth extraction, affecting about 2-5% of all extractions and up to 30% of wisdom tooth removals. It happens when the blood clot that forms in the extraction site either fails to develop, dissolves too early, or gets dislodged — exposing the underlying bone and nerve endings to air, food, and bacteria. The result is intense, radiating pain that typically starts 2-4 days after extraction.

How to Prevent Dry Socket

Do not use straws for 72 hours — the suction can dislodge the clot. Avoid smoking for at least 72 hours (ideally longer) — chemicals in smoke dissolve the clot and impair healing. Do not spit forcefully or rinse vigorously for 24 hours. Eat soft foods and chew on the opposite side. Follow all your dentist's post-extraction instructions carefully.

Risk Factors That Increase Your Chances

Smoking is the number one risk factor, increasing dry socket risk 3-4 times. Oral contraceptives elevate risk because estrogen can affect blood clotting. History of previous dry socket makes you more susceptible. Difficult extractions with significant bone removal carry higher risk. Poor oral hygiene and pre-existing infection at the extraction site also contribute.

Treatment If You Develop Dry Socket

Your dentist will flush the socket to remove debris, then pack it with medicated dressing that contains an analgesic (usually eugenol). This provides almost immediate pain relief. The dressing is replaced every 1-2 days until the socket begins healing — usually within a week. Pain medication and sometimes antibiotics are prescribed alongside the local treatment.
